LEARNING FROM PAST ERRORS
( Avoiding big accidents and catastrophes )
OBJECTIVE
Avoid the occurrence of big accidents in chemical companies,
learning with past accidents occurred in USA, Europe, Canadá
and around the world, that needed analysis from organisms
like USEPA, NFPA, CSB, HSE,
etc. Orientations to perform adequate hazard process
analysis. Presentation and iterative discussion of
many case studies with participants.
DIRECTED TO:
Safety engineers and technicias, production mangers ans
supervisors, process managers and process engineers,
researchers, projetct managers, and for trainees
training.
PROGRAM
1. THE GREAT CATASTROPHES IN CHEMICAL AND SIMILAR INDUSTRIES
Oppau, Texas City, Flixborough, Seveso,
Feyzin, Bhopal, Beek, Piper Alfa, AZF, Mexico City, Blaye
port terminal and others. Causes, consequences, law
modifications in countries wher they happened.
2. 20 IMPORTANT ASPECTS ON PROCESS SAFETY.
Important concepts for accidents prevention.
3.
ACCIDENTS BY NATURE
Discussion of accidents occurred in different industrial
activities: reception, storage, transfer, processing
and materials transport.
a) STORING AND TRANSFERRING SOLIDS AND LIQUIDS
- Accidents ocurred during handling of different products
and utilities. Loading and unloading trucks, railcars, tanks,
silos and warehouses.
b) HANDLING AND STORING COALS AND POWDERED MATERIALS -
Accidents handling processing and incinerating granulated
and powdered coals, in piles or silos, in industries,
refineries and in deodorization columns. Problems in
handling powdered organic and metallic materials
c) PROCESSES - Accidents during industrial operations:
reactions, solid-liquid charging, solid-liquid separations,
drying, milling an mixing products. Accidents dealing with
reactive processes.
d) PROCESS AND EQUIPMENT MODIFICATIONS - Accidents
caused by modifications introduced in industrial equipments
and processes. MOC.
e) MAINTENANCES - An important source of accidents
generation by lack of maintenance or during different
maintenance operations, in chemical industries and
refineries.
e) WATER, WASTE AND WASTEWATER TREATMENT AND VAPOR
GENERATION - Discussion of accidents during water
transfer operations, wastes and wastewater treatments, and
wapor boilers operations.
f) STATIC ELECTRICITY - Accidents due to static
electricity generation in many situations.
g) CONFINED SPACES - Concept of confined spaces and
accidents occurred in operations performed in these locals.
h) STERILIZATIONS - Accidents in pharmaceutical
industries during Etylene Oxide sterilizations.
4. REACTIVE PROCESSES
Underestanding reactive hazards and risks. Process safety
laboratory tests to asses reactive hazards. Safeguard levels
and equipments in reactive processes
5. EMERGENCY PREPAREDNESS
Management concepts and training for emergency preparedness.
Discussion of accidents and the need to be prepared to
emergency situations. Case Studies.
6.
PROCESS HAZARD ANALYSIS
Qualitative techniques to perform hazard analysis in
industrial processes.
